I understand that SJJF or it's partners are not liable for any injuries or accidents. Players fully understand they are participating at their own risk. Event will be video taped and/or photographed and that my, and my teams images may be used for future promotion of the game.

I understand my team must be a the Preliminary Games on June 15th and the Finals on June 20th to recieve or win the grand prize. I also understand my team must have 5 contestant to particpate and that if I don't have 5 contestants ready on time to particpate, my team will be disqualified.
